Special Dietary Needs

Once a child has been diagnosed with a food allergies or intolerances a Special Diet Statement form must be completed by an authorized medical authority, such as a licensed Physician, Physician Assistant, Advanced Practice Registered Nurse or a Registered Dietitian.

Updates to this form are required only when a participant’s needs change.

Lactose-Free Milk Requests

Zumbrota-Mazeppa Public Schools will provide lactose-free milk for children with lactose intolerance at no charge with a meal.  Students may purchase lactose-free milk a la carte or for milk break for an additional fee.  Parents can request lactose-free milk for their student by filling out a Request for Lactose – Free Milk form. Please note that this request must be kept current and submitted yearly.  

Zumbrota-Mazeppa’s Lactose-Free Milk option is “Lactaid”. Any other milk substitution requests will need to be accompanied by completing the “Special Diet Statement” link below.

Allergy Information

Zumbrota-Mazeppa Public Schools, Food & Nutrition Department works to ensure that Pre-school through 2nd grade menus are planned with no peanut containing ingredients. Zumbrota-Mazeppa Public Schools is not a "Peanut-Free" district as it is a public space, and food being brought into the building(s) cannot be controlled or restricted. As students enter the 3rd grade, children should have started working with their food allergy and the building nurse to ensure that a safety plan is in place along with an Epi pen if prescribed by a medical doctor.
